Before arriving for your first visit, please fill out the AMFD Medical and Dental History forms listed below. Please email the completed forms to amfdent@gmail.com prior to your arrival, or bring them with you at your first visit.

Once you arrive, we will take some time to get to know you, and review your medical and dental history. Prior to performing a comprehensive dental exam, we will take any necessary X-Rays*. Using this approach enables us to identify the best treatment path for your needs. Together, we will work with you so you can stay on the path to good oral health.
*We use digital X-Rays. This technology uses up to 50% less radiation than conventional film X-Rays.

If your concern is urgent and the office is open, we encourage you to call us for the quickest assistance.
Late & Missed Appointment Policy
Unlike some offices, when you make an appointment with us, we reserve time exclusively for you. We do not double book patients. If you are unable to keep your appointment, please contact us NO LESS THAN 48 BUSINESS HOURS prior to your scheduled visit.
If you are running late for your appointment, please call us as soon as possible so we can determine if we can still accommodate you.
